I work for an organization that has under it’s umbrella: Community Health Centers, Department of Corrections and Health Care for the Homeless Outreach. Myself and another physician complete the annual ‘provider evaluations’ for those working at the Outreach sites. We are looking for an evaluation tool that would best reflect the work that we do in outreach, as it is often quite different from the CHCs and DOC. Does anyone have an evaluation tool that they use that may be helpful?
